Overview
This schema describes a base, abstract element from which all personal health related documents derive. This means that all personal health related documents that extend the PersonalHealthDataType construct will inherit the elements described below. See the detailed descriptions of each document type for usage instructions.

PersonalHealthDataType
Base abstract type for extension of all personal health related documents.
Date- dateTime (optional)
- Abstractly, the date that the data was created at the provider. This is typically different than the date that the document was added to the Indivo server, reflected in the DocumentHeader and VersionHeader. More concretely, the Date field may reflect the date and time a lab test was performed, the date and time a note was written, the date and time an immunization was administered, etc.
Origin- ConciseContactInformationType (optional)
- The contact information for the institution where the document originated (typically populated when the document has been added by an external data source).
Comments- string (optional)
- Any free-text comments about this document.
Provider- ConciseContactInformationType (optional)
- The contact information for the health care provider who created the document (typically populated when the document has been added by an external data source).
EncounterId- string (optional)
- An identifier which is unique across an Indivo Record, used to identify and group documents which originated from a discrete health encounter.
