Overview
This schema models a health care proxy, a legal document that allows a third-party to make health care decisions in the event that the record holder is incapable of executing such decisions.

ProxyType
Date- dateTime (optional)
- The date this health care proxy went into effect.
Origin- ConciseContactInformationType (optional)
- The institution which provided the data in this document.
Comments- string (optional)
- Any free-text comments about this document.
Provider- ConciseContactInformationType (optional)
- Not applicable.
EncounterId- string (optional)
- An identifier which is unique across an Indivo Record, used to identify documents which originated from a discrete health encounter.
Proxy- ConciseContactInformationType (optional)
- Name and contact information of person designated as record holder's health care proxy.
Alternate- ConciseContactInformationType (optional)
- Name and contact information of an alternate person designated as record holder's health care proxy, should the primary proxy be unavailable.
PrimaryWitness- ConciseContactInformationType (optional)
- Name and contact information of primary witness to this proxy agreement.
SecondaryWitness- ConciseContactInformationType (optional)
- Name and contact information of secondary witness to this proxy agreement.
Image- ImageType (optional)
- Scanned image of a signed and witnessed proxy document, if available.
